Formally the levy applies to every foreign visitor. In practice checks at the border are selective and irregular — there is no hard 'no payment, no entry' rule.
Our practical view: paying takes five minutes online and closes the question. Treat it as a contribution to the island hosting you rather than another barrier. Checks are periodically stepped up, and being caught out is more annoying than spending five minutes in advance.
Do not confuse it with other payments
Three different things that get merged into one:

Where the money goes
The levy is earmarked: the province directs it to protecting Bali's nature and culture — beach cleaning and maintenance, upkeep of temples and heritage sites, and waste management programmes.
For a visitor it is the cheapest line of the trip: IDR 150,000 is roughly two coffees in Canggu. Set against visas, accommodation and transport, this is not the item to economise on.
